2. Healthcare Services

2.1 Scope of Services

Healthy Life Care Pros provides comprehensive healthcare services including but not limited to:

  • Preventive health care and wellness programs
  • Family and primary care services
  • Chronic disease management
  • Nutrition and wellness counseling
  • Mental health support and counseling
  • Telehealth consultations

2.2 Professional Standards

All healthcare services are provided by licensed healthcare professionals in accordance with:

  • Applicable state and federal regulations
  • Professional medical standards and guidelines
  • Evidence-based medical practices
  • Ethical standards of medical practice

2.3 Service Availability

Services are available during our regular business hours and through scheduled appointments. Emergency services are not provided through our standard practice. In case of medical emergencies, please call 911 or visit your nearest emergency room.

3. Patient Responsibilities

3.1 Accurate Information

You agree to:

  • Provide accurate, complete, and current personal and medical information
  • Update your information promptly when changes occur
  • Inform us of all medications, supplements, and treatments you are receiving
  • Disclose relevant medical history and current health conditions

3.2 Treatment Compliance

You are responsible for:

  • Following prescribed treatment plans and recommendations
  • Taking medications as prescribed
  • Attending scheduled follow-up appointments
  • Communicating any concerns or side effects promptly

3.3 Respectful Conduct

You agree to:

  • Treat all staff and healthcare providers with respect

4. Appointments & Scheduling

4.1 Appointment Booking

  • Appointments can be scheduled online, by phone, or in person
  • We will confirm your appointment via your preferred communication method
  • Appointment availability is subject to provider schedules
  • We reserve the right to reschedule appointments when necessary

4.2 Cancellation Policy

  • Patient Cancellations: Please provide at least 24 hours notice for cancellations
  • No-Show Policy: Failure to attend scheduled appointments may result in fees
  • Late Arrivals: Arriving more than 15 minutes late may result in rescheduling
  • Provider Cancellations: We will provide as much notice as possible for any cancellations

4.3 Wait Times

While we strive to see patients on time, wait times may occur due to:

  • Medical emergencies
  • Complex patient needs requiring additional time
  • Unexpected circumstances

We appreciate your patience and understanding.

5. Payment & Billing

5.1 Payment Responsibility

  • You are responsible for all charges incurred for services provided
  • Payment is due at the time of service unless other arrangements are made
  • We accept cash, credit cards, and most insurance plans
  • Co-payments and deductibles are due at the time of service

5.2 Insurance

  • We will verify your insurance coverage, but coverage is not guaranteed
  • You are responsible for understanding your insurance benefits
  • We will bill your insurance company as a courtesy
  • You are responsible for any amounts not covered by insurance

5.3 Billing Disputes

If you have questions about your bill, please contact our billing department within 30 days of receiving the statement. We will work with you to resolve any billing issues promptly and fairly.

6. Telehealth Services

6.1 Technology Requirements

To participate in telehealth services, you must have:

  • A reliable internet connection
  • A device with camera and microphone capabilities
  • Access to the required telehealth platform
  • A private, secure location for the consultation

6.2 Telehealth Limitations

  • Not all medical conditions can be diagnosed or treated via telehealth
  • Physical examinations and certain procedures require in-person visits
  • Emergency situations cannot be handled through telehealth
  • Technical issues may require rescheduling of appointments

6.3 Privacy and Security

We use HIPAA-compliant telehealth platforms to protect your privacy. However, you acknowledge that:

  • No electronic transmission is 100% secure
  • You are responsible for ensuring privacy on your end
  • Technical issues may affect the quality of care

7. Service Limitations

7.1 Emergency Services

We do not provide emergency medical services. In case of a medical emergency:

  • Call 911 immediately
  • Go to the nearest emergency room
  • Contact your local emergency services

7.2 Scope of Practice

Our services are limited to:

  • Conditions within our providers' scope of practice
  • Services for which we are licensed and qualified
  • Non-emergency medical situations
  • Patients within our geographic service area

7.3 Referrals

When appropriate, we will refer you to specialists or other healthcare providers for conditions outside our scope of practice or expertise.

8. Liability & Disclaimers

8.1 Standard of Care

We provide healthcare services in accordance with accepted medical standards. However, we cannot guarantee specific outcomes or results from treatment.

8.2 Limitation of Liability

To the fullest extent permitted by law, our liability is limited to the cost of the services provided. We are not liable for:

  • Indirect, incidental, or consequential damages
  • Treatment outcomes beyond our control
  • Complications arising from patient non-compliance
  • Technical issues affecting telehealth services

8.3 Medical Malpractice Insurance

All our healthcare providers maintain appropriate professional liability insurance as required by law and professional standards.

9. Intellectual Property

10. Termination

10.2 Provider-Initiated Termination

We may terminate the healthcare relationship for reasons including:

  • Failure to keep appointments

We will provide appropriate notice and assistance with transfer of care when terminating the relationship.

11. Governing Law
